How to Make an Appointment | Insurance Information

PatientsWe at the Donald W. Reynolds Senior Health Center believe in a team approach to your healthcare. As part of that concept you will be a part of the team of professionals focused on health maintenance and wellness. Your doctor and advanced practice nurse (APN) will follow your healthcare program. Since some visits will be with your doctor and others will be with your APN, your APN will coordinate your healthcare. Although your APN and doctor will alternate visits, your doctor will be responsible for your medical care. You and your family are an integral part of the decision making about your healthcare.

The social worker is an integral part of the team and will assist you, your nurse team, and your doctor to identify how you wish your care to be delivered. Our dietician will address any nutritional issues concerning your care. Our pharmacist, neuropsychologist, psychiatrist, and rehabilitation specialist will participate as members of your team as needed.

We believe the team approach is superior in the care and treatment of older adults. We look forward to designing a treatment plan compatible with your needs that will most effectively address your healthcare issues.

Insurance Information
The Reynolds Senior Health Center accepts Medicare assignment and honors most private insurance plans, including Medicaid. We will direct bill both primary and secondary insurance for your convenience. Please be sure to verify with your insurance company that services of our clinic are in network with your plan.

Managed Care and The Reynolds Senior Health Center
If you are covered by a managed care plan for your insurance, it is your responsibility to confirm with your plan what your coverage will be for any service.

At the Reynolds Senior Health Center, we deliver primary care to the geriatric patient. Many health plans or insurance payers however do not recognize our clinic as primary care providers but as a specialty clinic. For this reason, our physicians may not be available for enrollment as a PCP or Primary Care Physician.

In most instances, you will need to obtain a referral from the PCP that you enrolled with as a part of you managed care plan. Your primary care physician is typically identified on your insurance card along with your co-pay information. Please be sure to bring a copy of your referral to the clinic on the day of your visit. Without a referral, you will be responsible for the cost of services from the clinic.

Medicare and other Commercial Non Managed Care plans
The Reynolds Senior Health Center accepts Medicare assignment. This means that Medicare will only allow a certain amount of money to be paid for various services and procedures in which Medicare will pay 80% of the allowed amount, the remaining 20% will be the patient's responsibility unless you are covered by a Medicare supplemental policy or secondary insurance.

If your secondary insurance is a Commercial insurance, please be sure to verify with them that our services are in network with your plan.

Medicaid Customers
Medicaid customers may also be eligible for benefits and may require a referral from their Medicaid PCP.

ElderChoices
Are you trying to keep a loved one out of a nursing home? ElderChoices can help.

ElderChoices is a special Medicaid Program to help people stay out of nursing homes by providing services they need to stay in their own homes, with relatives, or with a caregiver. Many people who are not normally eligible for Medicaid may qualify for this special program. Those who qualify receive one or more ElderChoices services:

Homemaker/Chore Services Adult Day Care
Respite Care Adult Foster Care
Home Delivered Meals Personal Emergency Response System

Additionally, persons covered by ElderChoices also qualify for other Medicaid services, such as prescriptions and Personal Care.

Who is eligible for ElderChoices? Applicants must meet all of these criteria:

Age - Must be 65 years of age or older.

Income - Applicants' income must be no greater than $1,536.00 a month (2000 income limit). The spouse's income is not counted. For example, a husband and wife could each have an income of $1,535.00 and both be eligible if they meet other criteria. On the other hand, if the husband had an income of $1,537.00 and the wife's income was $750, she would be income eligible, but he would not.

Resources (Assets) - Applicants' counttable resources must be no greater than $2,000 for a single person and $3,000 for a couple. Unlike the Medicaid rule for nursing home residents, a couple's resources are NOT divided for this program. The applicant's home, household goods, and a car valued at less than $4,500 are not counted.

Giving away assets to qualify for Medicaid can result in a period of ineligibility. After a Medicaid recipient dies, the State will file a claim against their estate to recover the cost of their care. However, if the spouse or dependent is living in the home, it is exempt from estate recovery.

Care Needs - An ElderChoices nurse will assess the applicant in the home to determine whether the applicant needs nursing home level care. To qualify, an applicant must need help from another person with one or more activities of daily living (getting in and out of bed, mobility, eating, and using the toilet), or require supervision due to Alzheimer's disease or other dementia.
