Aging Initiative: Senior Health Clinic
The Senior Health Clinic, an interdisciplinary
primary care clinic, provides geriatric healthcare across
a continuum including outpatient, nursing home and the home
setting. The major focus of the Senior Health Clinic is health
promotion for older adults and their families through:
- improved access to care
- training opportunities for health care providers and the
general public about needs of the elderly
At the Senior Health Clinic, the elderly patient sees a team
of geriatric health care professionals. This team coordinates
the care for the patient according to the specific needs of
the individual. The core team consists of:
- Geriatricians
- Advanced Practice Nurses
- Social Workers
Other disciplines participating in care delivery may include:
- Pharmacy
- Nutrition
- Geropsychology
