HouseCalls
Program
The goals of the HouseCalls Program are to provide quality cost effective services to the homebound persons who are unable to visit the doctor’s office without a great deal of difficulty due to limited mobility, cognitive impairment and /or transportation constraints. Services include: physical exams, regular scheduled visits, prescription refills and referrals to appropriate services.
The benefits of the HouseCalls Program to patients, the hospital, and the community can be considerable. Patients benefit because at home care helps identify new problems and allows appropriate referrals to be made in a timely manner. The program also enhances patients’ quality of life and ability to live independently longer, and limits or delays the utilization of emergency department, hospitals, and nursing homes; it also allows the patient to live and die with dignity.
This service is available, under the direction of Dr. Delbra Caradine, Geriatrician, to individuals 65 and older and lie within a 30 mile radius of UAMS. Dr. Caradine provides primary medical care at homes while collaborating with traditional home health agencies.
This is not an emergency service. Appointments or referrals can be made by contacting 501-526-6387, Monday through Friday, 8:00 a.m. – 4:30 p.m. We accept self referrals as well as referrals from any other home health agencies or other healthcare providers.
Medicare, Medicaid, private insurance or private pay can be used to pay for the service.
