Home-Based
Primary Care
The goals of the Home-Based Primary Care (HBPC) program are
simple. An interdisciplinary team provides comprehensive primary
medical care to homebound veterans, as well as other services
including nursing care, rehabilitation, nutritional counseling,
and social work evaluation. As the turnover of acute beds
has increased, HBPC has become a key resource for facilitating
discharges of complicated patients and for supervising complex
post-hospital care. Besides Little Rock, there are two other
programs in Hot Springs and Pine Bluff which have proven to
be very valuable to home bound veterans who reside in the
surrounding rural areas.
